Although you will be bringing with you various records of your medical care, which likely contain a great deal of important information, I would like to also ``start from scratch,'' and go over your entire medical history myself. Sometimes this brings out important new facts and relationships between past and present medical events. It would thus be very important for you to complete the enclosed medical history forms, and bring them with you. Likely much of this information will already be contained in your previous medical records, in various places, but these forms will be very helpful for me, to be able to help you. It will allow us to discuss together all of the important aspects of your case while you are here, rather than my having to ``research'' your medical history. Also, when I write a report of your second opinion consultation, which you and your doctors will receive, it will be complete and factual.
I anticipate this will take no longer than 15 minutes to complete, and I thank you for helping me to help you.
